מסמך משפטי שנכתב על ידי נוטריון מוסלמי, חצי תחתון בלבד. מכיל שלוש הצהרות של עדים, מתוארכות 562 (?). בצד השני, הכתוב בכיוונים שונים, יש חמישה תוספות נפרדות עם אישורים, כולן מתחילות ב'אַעְרַפָּה' ומתייחסות לתוכן המסמך הראשי בצד הראשון.
Princeton Geniza Project description: Legal document written by a Muslim notary, bottom half only. Contains three witnesses' attestations, dated 562 (?). On verso, written in various directions, there are five discrete addenda with attestations, all beginning "aʿrafa" and referring to the content of the main document on recto.
