ככל הנראה אינו ממסמכי הגניזה. כתובת של איגרת בכתב ידו של יעקב חי בן אברהם מונדולפו מסיינה, המופנית אל הרבנים וחכמי הישיבה ("רזידנצה") של לונדון (לונדון, לונדריס). מתוארך: ט"ו באדר ה'תנ"ד = 12 במרץ 1694. ישנן הגהות בין השורות וכן ציטוטים בשוליים הימניים המפנים לפסוקי המקרא הנרמזים בגוף הטקסט. ראו גם מסמך נוסף הקשור לעניין (PGPID 19483).
Princeton Geniza Project description: Probably non-Geniza. Address of a letter in the hand of Yaʿaqov Ḥayy b. Avraham Mondolfo of Siena to the rabbis and sages of the yeshiva ("residenza") of London (לונדון, לונדריס). Dated: 15 Adar 5454 AM = 12 March 1694 CE.
