צוואת שכיב מרע של יפת הלוי, שנכתבה בדמשק. התיעוד אינו מתוארך, אך ראו בצד השני של המסמך. בצוואה הוא מקדיש שליש מבוסתן ובית הנמצאים בכפר חמורייה בקרבת דמשק, ומתנה שההכנסות מן הנכס ישמשו לרכישת מזון לתלמידי תורה. שני נאמנים מתמנים לפקח על העניין: יקותיאל ופתחיה. לפי גיל, מדובר במסמך מתוך אוסף תעודות ההקדשות היהודיים מגניזת קהיר.
