המחאת תשלום של הקודש. מתוארכת לשנת 1477 למניין השטרות, היא שנת 1165/66 לסה"נ. ההמחאה הוצאה על ידי שמואל בן סעדיה, בסכום של 9.5 דרהם, מתוך הכנסות חצר הירושלמים, לטובת אבו מנצור בן אברהם אלדמשקי. המידע מבוסס על גיל. ראו גם את אזכור המסמך אצל גויטיין. יש לציין כי גויטיין מצטט מסמך זה תחת סימן מדף שונה.
