מסמך זה כולל שני פריטים נפרדים. הראשון הוא מכתב תנחומים מאת גאון לנמען על מות בנו. הפריט השני הוא העתק של מכתב מאת האי גאון אל קהילת קירואן, ככל הנראה אל יעקב בן נסים. המכתב המקורי נכתב ב-11 באוגוסט 1006, וההעתק נכתב בכתב ידו של שמריה בן אלחנן. במכתב פונה הגאון אל הישיבה ומבקש להעתיק את המכתבים ולשלוח אותם לקהילות נוספות במגרב (המידע מבוסס על גיל, "במלכות ישמעאל", כרך 2, מס' 37). בפריט הראשון, הקטע השני מתחיל בסוף שורה 22 של עמוד ב'. הפריט השני פותח בקטע הראשון וממשיך בקטע השני החל משורה 53.
