מכתב מרופא אל דודו, ובו פורש הכותב את סיפורו העצוב. הוא מתאר את מות אשתו האהובה, נישואים שניים אומללים שעלו לו ביוקר, ולבסוף את בריחתו. הכותב מבקש מדודו למסור לבנו את חומש המשפחה. המסמך זכה לעריכה, תרגום וניתוח מפורט בעבודת הדוקטורט של עודד צינגר (פרק 5 ומסמך מס' 10), ונדון גם במאמרו "Narrating Masculine Failure" העוסק בעיצוב סיפורי כישלון גברי בחברה היהודית של ימי הביניים.
Princeton Geniza Project description: Letter from a physician to his uncle. Tells the writer's sad story, which involved the death of his beloved wife, an unfortunate second marriage, and his eventual flight. The writer asks his uncle to give his son the family bible.
